Mode Of Action

Cycloserine, an analog of the amino acid D-alanine, interferes with bacterial cell wall synthesis by competitive inhibition of two enzymes, L-alanine racemase, which forms D-alanine from L-alanine, and D-alanylalanine synthetase. these enzymes are involved in the incorporation of D-alanine into the pentapeptide necessary for peptidoglycan formation and bacterial cell wall synthesis.

Drug Indication Multi-drug resistant TB.
Precautions Obtain cultures and determine susceptibility before treatment, monitor haematological, renal and hepatic parameters.
Contra-Indications Epileptic convulsions, hypersensitivity to cycloserine, excessive alcohol.
Side Effects Convulsions, vertigo, drowsiness, headache, suicidal tendencies, character changes, hypersensitivity reactions.
Dosage Initially 250mg every 12 hours for 2 weeks increased according to blood concentration and response to max. 500mg every 12 hours; child initially 10mg/kg daily adjusted according to blood concentration and response.
